Network of Care for Seniors and People with Disabilities
Network of Care for Seniors and People with Disabilities was sponsored by a California Department of Aging innovation grant. The project is part of a broad effort to improve and better coordinate long-term care services at the community level.
Network of Care for Seniors and People with Disabilities is a highly interactive, single information place where consumers, caregivers, community-based organizations and county government workers all can go to easily access a wide variety of important information. The resources in this "virtual community" include a fast, comprehensive Service Directory, links to aging and disability Web sites from across the nation; a comprehensive, easy-to-use library; a political advocacy tool; community message boards; and many others.
The Network of Care for Seniors and People with Disabilities is now in more than 60 communities nationwide.
